Hammond Clinic's administrative professionals will work with you and your insurance provider to ensure the highest level of coverage and care. Please note the following related to HMO and Insurance handling:

If you have commercial insurance and have a question about coverage, please call (219) 836-5800, then press "3" followed by the pound sign, then press "1". For questions regarding your HMO coverage, please call the Managed Care Department at (219) 836-5800, then press "3" followed by the pound sign, then press "2". This department is available to assist with any difficulties you may experience regarding referrals or HMO information. If you have questions about Hammond Clinic billing or charges, please call the Patient Representative Department at (219) 836-5800, then press "3". Your rights to appeal decisions about your health care can also be explained. All medical records and inquiries are kept in strictest confidence; click here to review our Privacy Statement and Notice of Privacy Practices. Since the Hammond Clinic files your insurance claims automatically, you must provide updated and current insurance information. Do not present your doctor's office with insurance or disability forms. These forms should be submitted directly to the Disclosure & Disability Department, and signed by the patient. Insurance forms are processed in five to seven business days. Initial disability forms must be submitted in person.
