The Specialty Center is open 7 am to 8 pm Monday – Friday.
The Family Wellness Center is open 7:30 am to 8 pm Monday – Friday.
The St. John location is open 8 am to 8 pm Monday – Friday.

What are the hours of Urgent Care?
Monday – Friday: 8 am to 8 pm.
Saturday: 8 am to 6 pm.
Sunday: 10 am to 6 pm.
Who do I call for health questions after hours?
The Hammond Clinic has nurses available 24 hours a day to answer questions. Call 219-836-5800 and you will be prompted to an on call nurse.
What type of insurance does the Clinic accept?
The most common insurances that we accept, but are not limited to, are Blue Cross/Blue Shield Illinois, Anthem, Blue Cross/Blue Shield Indiana, HMO of Illinois, Humana, Blue Advantage HMO, United Health Care, Cigna, Medicare, and Aetna. If you have a different insurance, please call your insurance company directly to find out if the Hammond Clinic is part of your network.
Who should I call if I have recent billing questions?
If you have a question on a Hammond Clinic or Franciscan Hammond Clinic bill, please call 219-836-5800. If you have a question about a bill from Franciscan Physicians Hospital bill, please call 219-922-4216.
How has billing changed since the Hammond Clinic has joined the Franciscan Alliance?
Any procedure or service done before June 1st is billed by the Hammond Clinic. Any procedure or service done after June 1st is billed by the Franciscan Hammond Clinic. Any Imaging, Echo, Sleep Lab, or Alverno service done after July 5th is billed by the Franciscan Physicians Hospital.
Why do I always have to pay a co-payment?
Contact your insurance company directly. The co-pay is a part of your insurance company’s policy and the Hammond Clinic has no involvement.
How do I get copies of my records?
Come to 7905 Calumet Avenue, Munster, Indiana. Go to the front desk and ask for a Medical Records Disclosure form. Bring a copy of your driver’s license. The first 10 pages are free and additional pages are 25 cents per page. The request can take up to 30 days to process.
How do I find out more information about my doctor or nurse practitioner?

How do I get my results from a test or biopsy?
For Family Practice and/or Internal Medicine results, call 219-836-5800 and ask for the Results Nurse. For all other specialties call 219-836-5800 and ask for the specific department.
Are my bills covered if I am an HMO patient and I get ill while out of town?
Contact your HMO insurance company directly. If you have Humana, you are in a 50 mile radius of the Hammond Clinic and 30 miles if you have Blue Advantage. The Hammond Clinic is not responsible for out of town health coverage. |
